16. Insert a key frame on frame 25 of elements layer, then select the rectangle Tool from

the Tool Box to creat a button. Select the entire button, and convert it to Symbol ...

17. Name it "Backbutton" and form the type select Button.click ok.

18. First select the Button and give it the instance name "back_btn". Now double click

on it to edit the button.

19. With the Text Tool, write right on the rectangle "Back".

20.Right-click on the first frame and select

copy-frames ...

21. ... right click on the second, third and the fourth frames and Paste it.

22. Select the second frame and change the color for your Rollover Button.

26. Insert frame on frame 40 of both static text layer and also elements layer.

27. Create another layer and name it "Lables". Insert a keyframe on frame5 of lables

layer, then go to properties and name it "Form" ....

28. Do the same for frame 15,frame25 and frame 35 and name them "send","confirmed","failed".

29. create another layer and name it"send". Create a button for this layer. (sendbutton)

and name it "submit_btn"(see 15-23).

30. Create another layer.Name it "Date" and insert a Dynamic text field and name it " theDate_txt".

31. Create another layer and name it "action". click on the first frame of action layer. open the

Actions-frame(F9).

32. and enter the following script in.
